The consultant’s field notes
Dental compliance checklist for a records review
Start with the evidence, not the binder’s thickness. Use this checklist to organize a small US dental practice’s OSHA and HIPAA review.
This is an educational starting point, not a complete assessment or a guarantee. Confirm applicability, state requirements and practice-specific duties with qualified advisers.

Before you open any records
Confirm HIPAA covered-entity status and your consultant access. Determine whether the engagement requires a business-associate agreement before handling protected health information.
Prefer record locations and redacted evidence. Employee medical records have separate confidentiality rules; hiring a consultant does not automatically authorize access.

Exposure-control plan
Evidence to review
Find the written plan, exposure determination, latest review and safer-device evaluation. Look for documented input from exposed nonmanagerial direct-care staff.
Timing to check
Review and update at least annually, and when changes affect occupational exposure. Record the actual review date and unresolved actions.

Review date:
Bloodborne-pathogens training
Evidence to review
Check training dates, content, trainer qualifications, attendee names and job titles. A completion certificate alone may not contain the required record.
Timing to check
Exposed staff need training at initial assignment and within one year of their previous training. Exposure-changing procedures can require additional training.

Review date:
Chemical safety
Evidence to review
Locate the written hazard-communication program, hazardous-chemical inventory and matching safety data sheets. Confirm staff can access the sheets immediately.
Timing to check
Check training at initial assignment and when a new, previously untrained chemical hazard is introduced. This is not a universal annual renewal.

Review date:
HIPAA privacy records
Evidence to review
Review official designations, written policies, workforce training evidence, the privacy notice process and applicable business-associate agreements.
Timing to check
Check training for new workforce members and relevant policy changes. Document gaps without copying patient records into your task list.

Review date:
HIPAA security risk analysis
Evidence to review
Locate the practice-specific analysis of electronic protected health information, documented risks and risk-management actions. Check who owns each unresolved action.
Timing to check
HHS does not prescribe one fixed risk-analysis frequency. Review changes, incidents and periodic evaluation needs; an annual visit is a service cadence.

Review date:
Security procedures and follow-through
Evidence to review
Review access authorization, activity reviews, security-incident procedures and contingency plans. Ask for evidence of follow-through, not just a policy on file.
Timing to check
Confirm backup, restoration and emergency-operation procedures are addressed. Assign review dates based on the practice’s documented risks and changes.

Keep retention separate from review dates
OSHA bloodborne-pathogens training records must be kept for 3 years. Required HIPAA documentation generally stays for 6 years from creation or last effect, whichever is later.
Neither rule supplies one universal dental-chart retention period. Employee medical records have different requirements, generally employment plus 30 years under the bloodborne-pathogens standard.
Verify injury-log applicability before flagging a missing log. Also review equipment-specific duties, state dental-board rules and certificate requirements; these six records are not exhaustive.
Turn the review into a client book
Keep one dated action list per practice. Separate missing evidence from work not completed, then agree who will resolve each item.
Build a free review board, save one practice and keep reviewed evidence on file. Paid plans and automated reminders are not open yet.
